Our Community Living Project empowers people to make life choices based on their needs and interests. A person-centred plan is created with an individual which guides the support provided. We have extensive experience in working with people to address any complexities and challenges that may exist. This helps us to ensure that we can provide good practical support to individuals while ensuring that people have choice and control over their own lives.
Our support is very flexible, delivered at an agreed time to suit individuals, and covers a broad range of activities. This can include, but is not limited to:
- help with various aspects of running a home, including maintaining a tenancy, managing household finances and supporting the development of household tasks
- advice and practical assistance with healthy eating and preparing meals on a budget
- addressing issues of isolation and loneliness, promoting positive relationships with friends and family and supporting access to leisure and social activities
- help with personal care, support with prompting and administering medication and help with arranging and attending appointments
